Now that you have decided to purchase a hand-made memory quilt, there are a few things to complete to get the process under way! First, we ask that you please complete both the Quilt Order Form and Customer Agreement. Mail laundered clothing, both forms, and your payment to:

Customer Agreement
At OUS Quilts, we want to make sure you are getting the one-of-a-kind quilt that you want! But, as you can tell, we have to manipulate the clothing to fit each design, which alters it from it's original state. Click on the link below to fill out our Customer Agreement form to agree to these conditions so that we can create the custom quilt of your dreams!
Payment
We accept 3 forms of payment: Check, Money Order and PayPal.
